Parent/Guardian Consent & Declaration:

1.That the above-named children can participate in the O’Neill’s GFC An Port Mór GAA camp.

2. I consent to my child in the event of an injury to be transported by ambulance or car to hospital or doctor for medical attention where it is deemed necessary by the An Port Mór Coaches during the camp.

3. Photographs will be taken during the camp and may be used on the club social media sites and local newspaper’s; do you consent to your child/children’s photos to be taken?

4. Do you understand that if your child(ren) have any medical condition/allergy that the coaches should be made aware of this?

5. Do you understand that if your child requires medication; for example an inhaler; you must inform the camp co-ordinator?
